Living with either ADHD or PMDD can be a nightmare. Especially for those that are med-resistant. Living with both is comparable to living in a chronically high stress environment except, the environment is your mind - a place you can't exactly escape from.
The disheartening part... these conditions seem to intensify each other. Research increasingly shows a significant overlap between Premenstrual Dysphoric Disorder (PMDD) and Attention-Deficit/Hyperactivity Disorder (ADHD), with each condition influencing the severity and presentation of the other.
Seriously uncool. One or the other is bad enough. And while it may not be ideal, it's a reality for many.
Both conditions involve dysregulation of neurotransmitters (particularly dopamine and serotonin) which play crucial roles in your mood, focus, and impulse control. Because of this, there's quite a few symptoms that overlap.
During the luteal phase (the two weeks before menstruation), progesterone metabolites can affect GABA receptors in the brain. For those with ADHD, this hormonal shift can worsen executive function challenges, emotional regulation difficulties, and sensory sensitivities including rejection sensitivity that are already present.
How PMDD Impacts ADHD Symptoms
Executive Function Collapse: The brain fog and cognitive difficulties of PMDD impact ADHD-related challenges like planning, organization, and task initiation. These functions start to feel nearly impossible during the luteal phase.
Emotional Dysregulation Intensifies: ADHD already involves challenges with emotional regulation. Add PMDD's mood swings, irritability, rage, and rejection sensitivity....the emotional intensity becomes overwhelming.
Medication Effectiveness Changes: Many people report that their ADHD medications seem less effective during the luteal phase, possibly due to hormonal influences on dopamine pathways.
Sensory Overload: ADHD often already comes with sensory sensitivities. PMDD can amplify these, making sounds, textures, and stimuli feel unbearable during symptomatic weeks.
How ADHD Impacts PMDD Symptoms
Impulsivity Meets Mood Instability: ADHD-related impulsivity combined with PMDD's emotional volatility can lead to reactive behaviors, difficult conversations, or decisions you later regret. This can ultimately impact all of your relationships.
Self-Care Routines Collapse: The very coping strategies that help with PMDD (regular sleep, nutrition, exercise) require the executive function and routine-building that ADHD disrupts. They require the planning that ADHD makes very difficult.
Rejection Sensitivity Dysphoria (RSD): This ADHD feature can intensify during PMDD weeks, making perceived criticism or social interactions feel devastating. Watch out for those friendship breakups during the luteal phase.
TIPS
Track your cycle alongside your ADHD symptoms. Consider adjusting medication timing or dosage during the luteal phase with your prescriber's guidance.
Build in buffer time and reduce your commitments during your luteal phase.
Use alarms, visual reminders, and body-doubling for self-care tasks. Don't rely on memory or motivation during PMDD weeks.
During the luteal phase, prioritize low-stimulation spaces, noise-canceling headphones, and comfortable clothing.
before you go
Just a reminder that if you have both PMDD and ADHD, you're dealing with a compounding challenge that most people don't understand.
You deserve support systems, medical care, and self-compassion that both conditions require.
Understanding the PMDD-ADHD connection is the first step toward better symptom management. By recognizing how these conditions amplify each other, you can build strategies and finally release the shame that comes from not understanding why life feels impossible....at least half the time.
You're not broken. You're navigating a complex intersection of conditions that science is only beginning to understand. Go easy on you.
